Follow-up Session (20-25 minutes)

Any forms/documents that need to be generated, reviewed, and/or signed by Doctor (this excludes Disability Paperwork)

$50-$75 (based on length of paperwork. Furthermore, we are no longer doing any disability paperwork!)
